Thames Avenue Surgery is 'Open' to all new patients (as from 1st June 2020)

If you live in our catchment area (see map below), and you would like to register at our surgery, please complete a GMS1 form which is available to download from our website or you can request a form from Reception.

All new patients are required to complete a new patient health questionnaire and ethnic origin form (also downloadable from our website or upon request from Reception) and are invited to make an appointment with Angela or Vicky, our Healthcare Assistants.  This will give us an opportunity to meet you and to obtain important information about any past and on-going health problems.

Medical treatment is available from the date of registration.  Please contact reception for further information.

Please let us know if you change your address/telephone number as soon as possible so that we can keep our records up to date.  If you move outside the practice area, you will need to register with a new doctor.  If you are in doubt, please ask at Reception.
